York Outreach Recovery Team (YORT)

The York Outreach Recovery Team (YORT) are part of York adult community mental health services. They work with individuals to support their mental health recovery and help them gain skills to live as independently as possible. The team provide support to those who struggle to maintain helpful links with other services and who may benefit from a more flexible approach. They also work with people who are in hospital rehabilitation placements following the breakdown of their community living arrangements; these placements are usually outside the York area. YORT do everything they can to support people in their own home/community setting before a hospital admission away from locality is considered.
