I am pleased to notify you that Chatfield House ltd is a supported living accommodation, our home is based in Croydon. Our aim is to provide support for client with mental health and learning difficult needs in a supported living environment.

This supported living accommodation is specially designed to act as an intermediate stepdown between hospitalisation and independent living. Our aim is to develop our service users daily living skills and to increase their independence so that they can reach their full potential with the aim of fully integrating them back into society.

This supported living accommodation contains a communal lounge and two kitchen with a dining area. this home is located strategic position and in close proximity to mainstream amenities and public transportation.

At Chatfield House we: Provide support to individuals with Mental health and learning disabilities. Our environment offers privacy, dignity, independence, and choice, as our service users continue their journey to full recovery.

1. needs, who have been admitted to an inpatient ward and are clinically ready to be discharged, to be integrated back into society but have been unable to, due to barrier to discharge such as awaiting housing, package of care, and social care.
2. Provide a step down from hospital and for spill over as an alternative to unnecessary hospital stay when decision has been made clinically for discharge.
3. Provide 24hr support for vulnerable individuals with mental health needs, reassuring, monitoring their mental state, promoting independence towards self-administration of medication, liaison with community and home treatment teams for treatment continuity and support.
