We provide the following outreach services:
• Assisting service user to participate in religious and cultural activities and enabling the service user to attain a high degree of independence so that he or she will need minimum support with daily life from care workers and family members and friends.
• Assisting service user's to have access to education, employment, recreational or leisure facilities.
• Carrying out monitoring visits to check on service reliability and continuity and the well-being of the service user.

Please note the price shown is a typical cost for this service and is dependent on your individual needs and requirements. Once these have been discussed we can confirm a price.
