Description
Community Mental Health Teams (CMHT) provide community based support to people aged between 18 and 65 years old who are experiencing challenges with their mental health. They work with individuals to support their recovery and to help them gain skills to live as independently as possible. This includes developing an understanding of your mental health and any traumatic experiences that may have impacted you, exploring ways to manage your mental health, wellbeing and emotions as well as developing your strengths, coping strategies and problem-solving skills.

Referral to the CMHT is generally managed via GP, the Access to Mental Wellbeing Service or other allied professionals; people generally cannot self-refer unless they have had recent prior contact with a CMHT.
