Description
Our Outreach Health and Wellbeing service supports people with learning disabilities, autism and mental health needs to access a diverse range of groups, activities and social opportunities.
This can be anything from going for a coffee, attending a music lesson, or going to the cinema.
It could also be helping them to develop life skills such as cooking at home, travel training, budgeting or managing their household duties.
The service is delivered through 1:1 support sessions, support hours are unique to the individual and are arranged once the individual needs have been assessed
