Social Prescribing in York
What is this service and what does it do?
Social prescribers work closely with the NHS and other health and care services, to provide support that helps improve your health and wellbeing.
Often, people go to their GP for what is primarily a social issue (eg. loneliness, isolation, financial problems). Social prescribing helps provide people with an alternative to medical intervention, connecting them to support in their local community and across the city.
Social prescribers help you address the root cause of these difficulties, supporting you to make small changes that can have a big impact on your quality of life.
By getting to know you and understanding what matters to you, a social prescriber can help with things like managing long term health conditions, loneliness, isolation, low confidence, low mood and anxiety.
