YICT

Description

The York Integrated Community Team (YICT) helps keep people out of hospital and independent for longer by working directly with each individual to see if appropriate alternative solutions can be found. The team has been developed in partnership with local stakeholders to support ten GP practices based in the City of York.

The team offers continuous review and support which ensures that patients are optimised to stay at home. It'smade up of GPs, Nurses, Social Workers, Social Prescribers, Therapists, Paramedics, Health Support Workers, Dietitian  and Care Coordinators.

YICT work closely with the Rapid Assessment Team and the Rapid Assessment Frailty Area (RAFA) in the hospital.  Anyone with concerns about a vulnerable person can refer to YICT and speak to one of the team who will try to find solutions to help within the team or signpost to other services available.

Patients must be aged over eighteen years old and attend one of the following GP practices: Priory Medical Group, York Medical Group, Haxby Group, Old School Medical, Front Street, Dalton Terrace, Unity Health, My Health, Elvington  or Jorvik Practice .
