York Integrated Care Team

The York Integrated Care Team (YICT) has been developed in partnership with local stakeholders to support ten GP practices based in the City of York. The YICT helps keep people out of hospital and independent for longer by working directly with each individual to see if appropriate alternative solutions can be found. It offers continuous review and support which ensures that patients are optimised to stay at home. The team is made up of GPs, Nurses, Social Workers, Occupational Therapists, Paramedics, Carers and Care Coordinators. Referral to the team must come from a qualified health care professional such as a GP. Available for patients over eighteen years old that attend one of the following GP practices: Priory Medical Group, York Medical Group, Haxby Group, Old School Medical, Front Street, Dalton Terrace, Unity Health, Myhealth, Elvington  or Jorvik Gillygate.
